Baker Hughes Wireline Incident Aramco

Word from the wireline and oil industry street is that a RA source was lost/ left on site in Saudi. Company man found it put it in his in desk for days (romour us it was 5 to 45 days!)

Baker in scramble mode. Major sh#$ has hit the fan. Hmmm, wonder why the layoffs are kicking in....

Hard to believe it's wireline.... Maybe with the combination of MWD/DD, it could happen where the DD sc--wed up!!! Their RPS designation is a joke!

Purely engineer's fault. System is there in place and engineer was not following the procedure.

Baker LWD used to have a procedure for radiation work. Included in that procedure was rig floor survey and dispatch survey. Baker also have (should have) the RSO in the office responsible for tracking the radioactive sources.

how about making a checklist system. you take 2 sources to the rig site so you need to check in 2 sources when you leave the rig. Is this too much to ask?

You are looking at this the wrong way around.

Transporting things to and from sites is 2x the carbon footprint of just bringing them to the site. Get with the times. Baker is a forward thinking company and as such we are breaking the paradigm.

This is not the first time this has happened which means there’s something wrong with the system. That means something has to change. Your manager should come up with some ideas and if he can’t then he needs to be replaced.

Asking as a former RPS, how was this even possible? Your first job is to know where the sources are at all times. You scan the rig floor to make sure there is no radioactivity there, scan the source transport boxes and make sure the sources are in there. How did the AMO not notice a source not returning with the wireline tools
