Does anyone know what you need for the medical accommodation? I’m vaccinated but pregnant and a little worried for breakthrough- does that count?

I ve heard from someone going through medical accommodation that a Dr note alone is not sufficient. Bank request a hippa waiver from employee and making Dr fill out separate document for the bank to determine if they feel it is necessary to work from home...they are thinking that is an over reach...
After holding for an hour with employee relations they referenced a form needed by Dr in addition to accommodations form. I am just quitting today after 15 years. Just not worth the aggravation.
I've had a temporary medical accommodation in the past and all I needed was a note from the doctor. I have a friend with a medical accommodation for an undetermined timeframe and it is due to an emotional/psychological matter, and they still only needed a doctor's note.
I submitted one for employee and doctor note was sufficient. I don’t think you’ll get push back until everyone is back in office.

I went to doctor and they did a quick letter recommending to remain remote via mental health issues on hospital letter head. We will see if that's enough..
