On Teamworks, do a search for HR Services and Support. Then search for Covid-19 Support Request. Select the appropriate option from the drop-down, and complete the required fields.

I believe people are looking more for information on which specific illnesses are receiving approved on-going accommodations - not instructions on how to submit a request
Those who would want to request an accommodation are the same people who aren't smart enough to follow simple directions like this, so this doesn't really help anyone.
