I've been through this. In my case, I started Medicare in 2023, so things might have changed a bit, but here's what I did: Contact Alight at 1-877-722-0020. Select Health care. I don't remember all of the call-tree options, but if you say "other questions", and let them know you need a CMS L-654, they will get someone who knows what that is to help you. Not all of the Alight staff does. One person kept thinking I was talking about life insurance, but I was able to tell her I needed this form filled out and she got someone to return my call. Alternatively, you can make an appointment with Alight and a medicare specialist at Alight. I did that as well, but she was a few hours late for our phone appointment. But in any event, you give them what you need (for you, your spouse, etc.) and they will send the form completed with the Employer's portion to you. It took about 5 days for the completed form to be sent to me (it was sent expidited over UPS) from Alight. Alight, as a represntative of AT&T, fills out the second half of the form and will only fill in the last 4 digits of your social, so you fill in the rest. Then fax or deliver to a social security office. You should be signed up for Part A, at a minimum, or Social Security won't know what to do with it. You need the CMS-L-564 in order to get your Medicare #. I applied for Part B on line the day before and got the email notice that my application was under review before faxing. I included a cover letter in the fax with my name, Social Security Number, and purpose of Fax (CMS-L-564). Hope that helps.

I have been told by a Medicare Specialist it is not required if you target for it to start the first day of the month you are going to be 65. I retired from AT&T when I was 56 and I am researching all I can on Medicare.
1095-A is not acceptable to Medicare for some unknown reason. They require a physical signature and HR job title declaration.

I think the notice is called "Notice of Creditable Coverage". Probably need separate forms for health and prescription coverage.

No idea about who to call, but wouldn’t you be able to use the tax form 1095-A that you get every year with your W2?
What other form would even prove this?
