My boss asked me to reschedule something I took PTO for, in advance,because they didn’t want me taking time off to become a trend. My manager proceeded to take off two dynamic weeks after that.

At least the company didn’t mandate the experimental medical procedure. I’m beyond grateful that I got to keep my job and didn’t have to get injected.
The same point also highlighted prominently at the end of the Pulse survey results in FidCentral. Pathetic manager!
There is a 12 page long Dynamic Working FAQ that says managers should not tell employees that they can't take PTO during connect weeks. I would point them to the document and ask why they are giving guidance that goes against what HR has said.
Seems like your boss has personal vendetta with you or they're just straight up a$$h@les
