Medical Accommodation Forms

Can someone who has applied for a medical accommodation give a quick rundown of the forms you are supposed to get signed? Two people on here said their Dr. wouldn't sign them (liability?) and another said they just needed their manager to sign them. It would be really helpful to know what all is required before applying.

Consider carefully before you submit a medical accommodation form if you don’t absolutely need to have a medical exception. Schwab will most definitely target you for layoff and give you another reason why they are letting you go so they don’t have a legal challenge. Their goal is an almost complete return to office and you are blocking that path for them.

Raise the request with hr, I gave them info in the request.

They sent me a release form and a form for my doctor to fill out. I had them both filled out and emailed back to the HR business partner and my request was just approved a few days ago

First raise accomodation request with HR, they replied within 2-3 days and shared 2 forms to get it signed from doctor within 1-2 weeks of receiving the email from HR. Visit the doctor get the documents signed, submit the document, they approved my request after 3 weeks.
