If you give notice end of January and your last day with the company is early February, do you get insurance coverage for the rest of February? And How much PTO will they payout if you didn’t use any days in the new year?

Leave on the 2nd of the month or later to get medical the rest of the month. Don't leave on the 1st.

You will keep your insurance for the full month you quit. If you quit early February, you'd have it through February 28. PTO is paid based on the HR table for what's been earned. You could use caregiver days.

You get nothing. Once you are out, you are cut off 100%.

There are tables for how unused vacation gets paid out - you can find them in HR OneStop. It’s basically proportional to how far into the year you are, so you’ll probably get paid out for 2 or 3 days.
