Let’s say you had 1 holiday and 2 PTO days in a week. Does that meet your 3 day quota? Or how does their methodology account for these days?

Not true. If so then one holiday and one PTO day meets three days in office.
What is true is if you take four hours PTO OR more than you meet an in office day for that day.

Depends on the holiday
A better question is, "Why ask on an anonymous internet forum instead of to your management chain who will be acting/reacting to that use of time?"
Holiday's and PTO are counted as in-office days, so you would not go into the office that week.
Holidays are "in office days".
