Thread regarding Chevron Corp. layoffs

CNAEP. RCA? Where is the lesson learned? Maybe the NEXT TIME !

First it takes years to master the Why Tree not a class given to a group of young inexperienced kids or displaced professionals that finds themselves in the II&R department. The litmus test of great RCA's is that you don't have the same incidents happening over and over....at GOMBU take a look at our RCA's and the same things happen over and over. Inexperienced facilitators stop at symptoms not systematic causes. Management system root causes severs the weed at the root never having it grow again causing the same incidents This is wide spread throughout Chevron. Not just GOMBU. Chevron has a liability when they have a document that continuously points out symptoms and incidents keep occurring. Pull out the RCA's and you will see the same incidents happening over and over......it's madness. You will be bullied, marginalized or fired if you do this. That's why Chevron is failing at all levels. Even during the process no one indicates a,problem as the OP said you will be labeled. There are many many great systems other than Why Tree. We don't have the experience at any level at any BU in II&R. Eventually the symptoms that are not root causes lead to fatalities. You also loss off everyone involved in the process as they know it's not going to fix anything. The process creates useless and I pick my words carefully....and dangerous action items. There is a good saying that applies to Chevron in spades. Make reporting and incident much more painful that not reporting and you will find no one will report. Why would they? The process actually causes incidents and promotes fatalities. Again I choose my words carefully.
