depends on your country, length of service, age and how much you negotiate.
usually 1-5 years get the same compensation...this can be up to 3 months salary and payment in lieu of notice there may even be another additional amount for your age
if you're in a country with strong protection (Norway) your payment in lieu of notice may be 3-6 months. Depends on your contract
Some LRs will happen after shares vest in which case you get the vested shares, others will happen before they vest. This is likely down to country law. You won't get any shares that haven't vested.
In the US you should get an option to extend your health care but this will be at your expense.
