Additional graduation from which dept and when? Stay Healthy is HAH...maybe we are talking about the same 'graduation'? What is NP??

Newsflash: They are also "graduating" a large number of HAH members to "telephonic" and NP programs. Does anyone have these stats please? Maybe we are all getting ready for the unemployment line.

I was told by my coach that by "graduating" all these members, it is a testament to how well we have worked with them and empowered them to manage their health/conditions. That is the positive spin they are trying to put on this. If you figure each PHC's roster at full capacity is about 700 members, we are graduating the equivalent of about 26 PHC rosters.

I notice they are now asking to see who speaks multiple languages. Last week (or was it the week before?) they were trying to find out who has a license as a social worker. It's like they are trying to find reasons to KEEP people. Something is definitely up, and I have a feeling at the beginning of May (just before the Town Hall meeting they rescheduled) the axe is going to come down.

Yes, I am actively looking for other employment. Enough is enough.

Holy sh--ake mushrooms. How many people are IN the Stay Healthy program?? I love the insight this site offers, but sometimes wish I was till living in the dark. :(

May 26 - over 18000 members will be released from an already small program. I don't think anyone actually believes this is a kind and generous effort by Humana to allow associates to catch up on alerts. It is more of the spin and lack of transparency we are accustomed to - nobody even questions it at this point. We are all just pretending everything is just fine.

When are the Stay Healthy members 'graduating'? You can only catch up on overdue alerts for so long. With no new members coming in, I'm bracing for the worst.

Stay Healthy is part of HAH, assigned to care management for a healthier population than HCCP members. Our coach did tell us today that SH is not enrolling new members at this time. Thousands are also "graduating." We were told this would allow us to catch up on our rosters and address overdue alerts.

I am in SNP and know nothing about Stay Healthy. That's not Obamacare is it? SNP began "transformation" in 2015 and "optimization" over the last year. What exactly is Stay Healthy?

I am currently a SH PHC. I'm not sure what is going on, but there are a lot of sudden changes with our rosters. I recently had over 100 people moved off my roster and reassigned to someone else. They are also about to "graduate" a lot of members from the program because they are deeming them "too healthy" (CBA score below 8750). If they are not going to be enrolling new members and refilling our rosters, I am deeply concerned SH PHC's are next on the chopping block.
