Can someone direct me to CMS guidelines related to care plans? There is some debate on our team about whether they really intend for us (phc’s) to do care plans, or whether they might just get rid of us altogether.

I plan on checking out cms.gov myself, but if anyone has the link to care plan requirements, that would be helpful.

In my honest opinion, I don't think this program will survive much longer. I believe they are stringing us along until they know for certain the direction they will take the program. The nurses may remain but I am not very hopeful for the other disciplines outside of them.

Below poster, do you know that they need phc’s for the next phase or is that just based on the 2.0 training talking points? Because the phc’s have been fed a line of bull all year and then keep getting cut.
