I go to my doctor regularly and never report my numbers to Frank. They gather high-level statistics to find ways to tamper with the insurance plans to bilk associates to the maximum degree. Shameful regime.

When there is financial incentive attached (ie: reduced premiums), for most people that essentially means "mandatory."
Fiserv does offer mental health, it is called the EAP and part of your standard benefits, do not need to be enrolled in the medical plans to use it.
Also the biometric screenings are optional
It;s actually part of the ACA Law. Not mandatory but if you participate you can get points on Red Brick and reduce premiums. I've been on my spouses insurance for 4 years but that's how it was when I have Fiservs.

I don't smoke but I always found that one to be a little intrusive.
I had my endocrinologist do my physical in early January, 2022. Heoght, wight, A1C, etc. The results were uploaded to Fiserv per the company process. I trust my doctor more than Fiserv's provider. Sobfar not one word from Fiserv.
I have seen self insured Fiserv fire two people with cancer.
The question should be what are they doing with the data. Most likely will be tied to your HR file in some way to dump unhealthy people, similar to how they dump experienced staff to hire cheaper labor. Be lucky they don't require a mental health check. It would just be another way to label you and mark your file for layoff.
Be lucky they’re only looking for a small sample. Frank might try and sell several pints and one of your kidneys next.
