ProPublica published an extensive report stating that IBM:

Denied older workers information the law says they need in order to decide whether they've been victims of age bias, and required them to sign away the right to go to court or join with others to seek redress.

Targeted people for layoffs and firings with techniques that tilted against older workers, even when the company rated them high performers. In some instances, the money saved from the departures went toward hiring young replacements.

Converted job cuts into retirements and took steps to boost resignations and firings. The moves reduced the number of employees counted as layoffs, where high numbers can trigger public disclosure requirements.

Encouraged employees targeted for layoff to apply for other IBM positions, while quietly advising managers not to hire them and requiring many of the workers to train their replacements.

Told some older employees being laid off that their skills were out of date, but then brought them back as contract workers, often for the same work at lower pay and fewer benefits.

The layoffs may appear to be targeting older employees but it is not age it is grade and salary. I received a listing containing title, age, and grade that document verified my theory. Two weeks after my layoff my position was posted two grades lower. The writing was on the wall for me when my leader told me two years ago I was one on the highest paid members of my team therefore no raise was awarded. I actually felt ashamed of myself (stupid)! I gave them my all and then some I was worth every penny I earned!

Count me in for a class action suit. This company is ruthless and does target older workers. I was told the day of my lay off with no notice.

Has anyone started a class action lawsuit? I was only given 30 day's notice. Anyone know some good lawyers who have won some high profile class action lawsuits??? Let's get this ball rolling!

I did not get a 60 day notice, I was told the day of and if there is a lawsuit, I will definitely be a part of that.

I was laid off at UHG 4 years ago after 13 years of dedicated service. Because I was over 40, they provided me with a list of others laid off and their ages (no names attached). I believe it was provided with my severance package. So I know they have done that with past layoffs...

I was fired in 2018 during a rash of firings that all had one thing in common, we all either had disabilities or were approved for intermittent FMLA. Every one of us was given a bogus reason for our terminations, mine was that I violated the integrity value. However, it was determined that I was fired without just cause by the state, as it was determined for each of the other employees terminated under similar circumstances.

I’m in on any law suits I will speak up I’m one of 16 who got laid off today 10/2/19 I understand they are in violation of WARN ACT !! Been there 6 years and I’m 29 currently. Am I bitter no but this isn’t right!

I don't know anyone who even asked for the age information when they received their severance. I'm pretty sure most people aren't even questioning the package, just grateful to get one. And that is what UHG banks on. I'm certain also giving laid off workers X amount of days to sign and return the package is considered "under duress". I will be looking at mine and consulting a professional prior to signing one if I get laid off. The people complaining about WARN and age discrimination aren't helping themselves or anyone else by being silent. In the IBM case, the actual severance documentation was found to be in violation of the EEOC. All of the bullet points the OP listed are known in UHG. #2 is Optum's Freshers /outsourcing. #3 was the VSP offered last year. #4 is a known fact and has been posted on this site previously. #5 is a standard practice in the MN office.
