How many more derails before the FRA finally does something

The regulatory apparatus (including the FRA) has been systematically dismantled over the past four years.
Rule of thumb: if the lobbying power of an industry or company exceeds the regulatory power of the government agency tasked with regulating it, then the industry or company will get most of what it wants most of the time.
The only way the FRA is going to get involved is if the acceptable amount of deaths is exceeded. Unfortunately only the actuaries really know what that number is.
Correction it is Fictional Railroad Agency (FRA) haha
Pretty sure the FRA is a fictional government agency.
