All the above is true. You also qualify if you are retirement eligible. I got mine after I was laid off in June because I met the retirement eligibility criteria.
Employed by the 15th, it will be deposited into your 401k on the 30th
Yes -they should match as you are required to be employed up to Dec. 15th to qualify for the match.
