You've GOT to be kidding. THIS is what you're worried about? Find a new job.
The form? Oh, yes, there is a form. But you'll never find it.
In the past year, its been moved around 6 different Sharepoints, each one of which has been obsoleted, and replaced, with yet another Sharepoint.
If you do win the form lotto and actually find it on some obscure database, you will be submitting it to some unknown entity, who probably got laid off in one of the "right sizing" exercises" that have been conducted over the past couple years. So you will never hear of it again, once you hit the "submit" button. Into the vast, disjointed beyond any hope of understanding, Ford electronic web vortex, it will go.
So, good luck.
Your best bet would be just to bring the critter in, anyway.
Just play dumb if some self important, middle manager dweeb, executive wannbe, gives you sh*t about it.
Tell him that you got permission from his boss.
These types live in dread fear of questioning their superiors, and stepping out of line.
Will at least keep him up at night for a few weeks, wondering if he should say something to his boss, and risk the future executive job, which he will never come close to getting, but believes that he will if he only s—s up to enough higher ups.
Or just pretend to work from home, that is a commonly played game.

You don’t need a form for Happy hour at work. That is a standard perk.
At least it was in the Mainframe Infrastructure area.
From 11-1 was happy hour at the neighborhood bar.
Everyone knew that no work should be done after “lunch”.
People either slept or socialized from 1-3 and then left for the day.
And yes the LL6s were at the bar also.
Those GSR declining to participate were ostracized.
