Has anyone requested extended unpaid leave? Any advice or experience with the process would be greatly appreciated. Thanks

Make sure you give a good sound reason. To be approved you have to be in good standing AND be someone your manager wants to come back and THINKS you will most likely come back. They only approve for one year, you have a chance at getting a 2nd year approved, but don't count on it.
You get healthcare at same rate during the time.
MOST IMPORTANT: There is NO guarantee you will have a job to come back to. Your former manager generally has 1st dibs, and your return can happen to any dept that can accept an internal transfer if your old position is gone (ie they don't need an external hire ticket, just the headcount which is different).
Ask your manager to submit LOA (Leave of Absence) if they agree to provide you with extended unpaid leave. Another option would be contact your HR partner. Do a search in w3 for HR bot and request for HR partner contact details.

Ask your manager, if it's medical then can leave immediately. If it's not medical you need to give 2 weeks notice, but more is better.
