just email your supervisor / manager
make them do their job to ensure that you get what youre entitled to and that the process goes smoothly
thanks for your hard work and good luck in your next endeavor

OP here, I actually didn’t go through the PDS as I was on sick leave from irreversible health complications caused by this same work.

Why are you leaving? Don't act out of feedback from one year

No need to include HR. Your vacation will be paid. Just be sure and confirm the hours owed to you with your supervisor (I would get your admin to print it out from the system).

Ensure your primary email on your XOM accounts (benefits, 401k, pension, etc) is updated to your personal email. They won't do it after you leave.

Get all your annual checkups done (dentist, vision, bloodwork, etc).

Best wishes!

Be sure to use all your employee benefits. Be sure that you leave no money on the table.
