Why do some of you not understand it’s not about getting the vaccine, it’s about providing personal information to AT&T that is NONE of their damn business? Particularly for someone who does not go into an office, EVER. Some people still believe in personal liberties and privacy. There are plenty of vaxed managers who do not feel the need to share that status with the company.

What religion are you a member of that does not allow vaccination?
File for an exemption. I go look on how a religious exemption might work, and with an American search engine, I come up empty handed.
I tried an overseas search engine (Yandex) with the exact same search terms (vaccine requirement religious exemption example), and the 1st hit is:

State law likely has control over religious exemptions.
Well now that its approved by the FDA, the company can be held liable for mandating the vaccine, if you have any adverse reactions to it, as the emergency statues of the vaccine issued by the government is removed.

To answe yuur question. On HR OneStop there is a vaccine exception form. You can request either a medical exemption or a religious exception.
In both cases yiu need to provide the accommodation you seek that will enable yuu to continue your work. For example if yuu currently WFH request to continue to do so.
Medical exception may require reauthorization where as religious exception will not.
