At what point in the hiring process do you ask if your already approved medical accommodation can be accommodated by the hiring manager? I’m about to apply got a job and want to be prepared for how to handle this.

The managers don't really have a say in it. It's an HR Work accommodation. If they were to say no to anything medical it would be a world of legal battles for WF. Even when people request LoA managers aren't really involved except to get back-up people to ensure tasks are still completed on time.
well if hr says yes and your manager says no then that puts wf in aj n actionable position. He always trump's your manager.

I have a medical accommodation to telecommute full time. I've stopped applying internally as hiring managers simply say I do not live in a "go forward" market/location and am then deemed unqualified. If I lived close to a "hub" it may be different. So, I am applying externally in hopes I can continue to grow and provide value in a career.

Even though HR approved it, the persons current manager still had to agree to it. So they are asking what happens now if they get a new manager through hiring process.
On a serious note though - disclose it whenever. They aren’t going to care. As long is it doesn’t come across as you’re lazy.

if hr has approved the accomodation then there is nothing rhe hiring manager or even your current boss can do

Yikes, not even close. It has nothing to do with missing work. It’s a medical accommodation for telecommuting. The entire team is spread out and not in my state so there doesn’t appear to be any burden on the team for that.
Is the medical accommodation one that allows you to unexpectedly miss work at any moment in a retail environment where you can on any given day choose to frustrate a group of 10 people by not showing up to work?
If so can you please help me understand your perspective. I don’t think you can imagine the damage you will have on your manager’s life if that’s the case.
