Definitely was mid level. Not s-x . Finance. Related . And at least three SVPs in IT were aware and signed off

Highly doubt it was mid level given that it was discussed opening at 2 different sessions lead by leadership. Mid level terminations don't get that much attention.

The Code of Conduct is ONLY for mid level managers and below. EVERYONE knows of more than a couple of handfuls of stories about execs and their tawdry office relationships. It is only when it becomes too well known that anything is done: "Rules for thee but none for me!"
