What does the doctors note need to contain? I know it was a few statements, but I can’t recall what information human resources will need. Thx!

the accommodations department sends you an email with exactly what the letter needs to contain and a form you can use as an alternative to a letter.
If you need to ask, you probably don't qualify.
Keep it simple. Mine said she treats me for said condition and believes I should be allowed to work remote going forward with no end date.

HIPAA is that a medical provider cannot disclose info without your consent. It has nothing to do with whether you can be required to disclose if you want an accommodation. You choose whether your need for accommodation outweighs your wish not to disclose.
to the OP's question - doctor's offices are familiar with this kind of stuff. they have to fill out a form. it has to say what your condition is, and why it makes you unable to commute/work in an office environment. It can rise to the level of a formally defined "disability" (which has specific meanings under ADA) or it can be something that isn't a disability but in the doctor's opinion, would make it a significant hazard to your health/condition to go into the office.
just stop going in and when they ask be like didn't my doctor send it to you? I gave him you'r email address...
do you even know the HIPPA guidelines? While they prevent your doctor or the company from sharing medical info about you without your consent it doesn't stop the company from requiring you to provide proof of whatever illness you are indicating you need an accomodation for. That may be as simple as one sentace stating you are diagnosed with XYZ and should refrain from certain activities, but if they need more info after review you will need to provide it or risk having the request denied. it is also worth pointing out that the accomodation doesn't necessarily have to be working from home. They will work with you and your manager to determine what it should be.
all that needs to me disclosed on Doctors note is illness.. They cant cross HIPAA guidelines.
