Lol

Moderately severe depression and severe anxiety…. In other words a typical BAC employee. I will answer my own question with a little more research

Is this a standard HR form, or a government form? If HR form, is it linked on flagscape?

Easy peasy. I took time off last year for this very reason. I wish you mental health.

No, no one has to know the reason except for the person who approves your medical accommodation. They don’t even have to know it has to do with mental health vs. a physical ailment. Depending on your manager they can try to get some info out of you but they are given zero information from HR. They’re only told what the accommodation is and when it starts and ends.

Do you have to tell your manager you diagnosis? I don’t want to share with my direct manager.

Yes! My psychiatrist filled mine out for a year. I’m full WFH. It was approved with no issues. Just need to have a new one filled out next year. Only issue is if your mental health doctor isn’t willing to fill out your form.

Yes, I am. Very easy to get approved, my therapist just had to fill out that2 page form. Approved to wfh 5x a week for a year. Will have to renew once that year is up.
