Medical accommodation process

What’s the process? Do I have to tell my boss the reason? My doctor is willing to complete paperwork and my disability is covered by the ADA. Is it hard getting approved?

Any thought this is easy peasy I think is ridiculous. having been considering it, I know it won't be. you need a plan, especially if you don't have any active medical engagement with therapist or shrink who can do the paperwork once presented. Reengaging a shrink just for this purpose will be seen for what it is, and that won't go well either. There's no easy peasy here. Question is how much is it worth to you to pursue, then form a workable plan if benefit outweighs the cost.

My physician wrote exactly what HR requested and they accepted it. THEN, 5 hours later they rescinded it. The Accommodations Team is filed with contractors that have zero follow up skills. Our SVP HR Partner just continues to push me back to the Accommodations Team. Don’t get me started on Management.

I’ve hired an attorney. I can’t deal with this clown show.

If it's because you can't drive, doc will also notify DMV.

Super easy and painless. Get the paperwork from bofa. It’s a two page form with boilerplate questions, your dr should fill it out in a few minutes. Email it back and starting from the time you submit you can wfh.

Mine was easy. My doctor thought with all my under lying health issues it would be crazy to go back to the office. I always try to make my doctor's job easy. When I have to renew my work at home form I'll give a copy of the last one to just copy and sign. My original work at home approval has no end date usually accommodations have a 1 year end dates.

Go through flagscape for HR. There is a way to submit that. HR will notify your boss but will not disclose the issue, if you don't want to have it shared. They will just ask for supporting documentation, which you can get from your doctor.
