Too much hubris. Management that is close to zero point on x-axis of Duning-Kruger curve can’t understand ideas from knowledgeable engineers. Engineers get frustrated, management thinks engineers are problems, and innovation suffers. Intelligent people who know how to lead people aren’t promoted, only the ones that can bury their noses in orifices do.
