Remember a couple of years ago when in Nigeria the flare air blower exploded. There was supposed to be two but one had been inoperable for years to save money. Did any heads roll for not spending the money to maintain critical safety devices or did we just blame the wrench turners who burned themselves like we usually do? Was the person in charge of the facility promoted for achieving such low OPEX? I love these guys who basically run the plants to failure and then hope they get promoted to their next job before everything falls apart.

Yep, 2x100% redundancy. Never bothered to repair the failed one. When the other one failed, top down pressure from the Senior Production Manager, little smarmy dude. Of course he had "plausible deniability" of any interactions when corners were cut to bring production back online. Someone got ki-led and they decided to spend a bit of money to get both blowers going. No heads rolled.
Smarmy dude eventually moved on, not to greater things.
