Yes. You need to send in the healthcare provider certification form for the dates you'll be out. It doesnt have to be the company provided forms either. The healthcare provider can use their own form and T has to accept it.

If it’s for maternity leave, AT&T’s IDSC is pretty helpful with answering questions. They run the disability services, but are used to getting more general questions about the different types of leave you have to string together in a specific order. It can be complex.
