You could use the free app My Contacts Backup, which has the option to export your iPhone contacts as a csv (Excel) file which can then be printed. It also will backup to a vcf file, which can be imported to another iOS device or mac Address Book.

Just enable iCloud for contacts and you will be able to look them up online after your phone is turned in and deactivated.
You should be able to keep your Apple ID even if it was linked to COP e-mail. Apple IDs can be changed as well. Make sure you have a recovery e-mail address set that is your personal one.
