Does Intel pay to the COBRA administrator directly ? Or Intel just pays us a lump sum and then we make monthly payments to the administrator ?

intel pays for 6 months.
So we have a choice of medical, dental, vision etc. Does Intel pay all of them for 6 months, or just give us a lump sum to cover it ? If the latter I'll drop the vision and probably dental to save a few bucks.
There's a "Health For Life" option also. Does it include using the GYM facilities ?
Sorry, my link didn't work, it's titled Flexible Spending Accounts (PDF, 109 KB) under the plan information tab to get the details of the Health FSA (aka YSA).
Did this just now online too via the Hewitt link. Gives you the options for Medical, Dental, Vision, Employee Assistance Program and the Health FSA. Unfortunately, the dollar amount you choose for the Health FSA can't be changed to a lower amount except for 0. So I got to choose between 0 and 2250 (what I had chosen last fall) only. Bummer, so I choose 0 since I had buffered with that allowable 500 rollover which is useless now. I'm not forfeiting any money to them! Yes, thank you for the link and all the information. You do not need to wait for the paperwork. When I logged in, a COBRA Enrollment message with the deadline was waiting for me.

You can also enroll for Cobra through the My Health Benefits web site (Hewitt).
Are we supposed to receive instructions with the phone number first before calling? Would COBRA know if you are eligible or if you get the 3, 6, or 12 month of coverage?
Do we need to call to enroll ? Or Intel is supposed to send us the paperwork first ?
