Unbelievable scrap rates and low yields have long been the status quo with little real engineering done to solve fundamental design issues contributing to low production yields. So long as revenue is high with a descent profit margin is realized no real breakthrough technology changes are realized. Real change agents are viewed as malcontents and dissidents and either leave looking for real challenges beyond HOS's stifling grip or succumb and comply and accept mediocrity. There are companies out there that and foster a positive work environment and develop their employees to succeed. 3M has that environment.
