Care plan training pushed back

Anybody else get informed that care plan training will be pushed back till January?

To PWFaUHA-1dfa:

In light of this week's layoff announcements that included many PHCs, do you still want to tell me not to "read anything into" the postponement of PHC care plan training that was supposed to start next week?? Again, nothing to do with the holidays--that was an excuse--the leaders knew this layoff was coming.

I am in the care plan pilot and as they rolled out cgx 2, with people on the teams pairing up to work together through scenarios, that is what they are hoping to do over a two week period. The issue is holidays, time off and coordinating it all, since it's still being piloted. Please don't read into it

So, my PHC team was finally informed today that our care plan training is postponed. Ominous sign. I don't believe it has anything to do with the holidays because the training was supposed to be web-based, not with live trainers, I am more worried now about the future of PHCs. This whole week has felt like a prelude to a funeral. I'm convinced now that the Nov. 1 layoff date is true.

It would be interesting to see what requirements CMS has laid out for chronic condition care plans / care management. I’ll check that out on cms.gov when I have more time.

Poster, you said :Unlicensed staff are not supposed to educate either. I don't think you need any other proof that any unlicensed PHC is going away. The only ones that will be left are the HCMSS that went into the role because they aren't master's level ss.

That doesn't make sense because there are those social workers in that role aren't all licensed. I think that's why they are called coaches....that's the education they do

I think you are wrong, no disrespect, but the role of the PHC, if you read the description, involves education. They can't educate on nursing issues, but they educate on so much more, because our members have more than just nursing issues.

Unlicensed staff are not supposed to educate either. I don't think you need any other proof that any unlicensed PHC is going away. The only ones that will be left are the HCMSS that went into the role because they aren't master's level ss.

If this is true, it makes more sense to me why every other health insurance company requires a license. I was wondering why they don’t have unlicensed roles like Humana does. Interesting discussion.

Yes, same template, different choices, they are not medically based choices, they are based on the key phrases assist, educate and coordinate, just like we all do now, so as the previous poster said, it's a different format to put our usual note in

Our care plans are not traditional nursing care plans Scope and licensure would probably have no bearing

Actually care planning is out of the scope of practice for unlicensed staff. Care plans are direction of care, and unlicensed staff are not supposed to direct care. Unlicensed staff can't legally direct care, or care plan, because they cannot legally assess. And to make a plan of care you have to be able to assess a patient and their needs. It is very much outside of scope of practice for PHCs, unless they are licensed such as the HCMSS that went to be PHCs.

Oh, yes, they do. That question was specifically asked in our Live Heathy town hall meeting earlier this month, and the response from JR was "We don't expect that to change much based on the pilot." Great! Thanks a lot pilot people!

Actually, PHCs have always been identifying needs and creating goals and action steps with members to close gaps in health care. We just document that in a regular note instead of a care plan template. There's nothing about a care plan template that is out of our scope. We will document the same work we've always been doing with members, but in a different format to meet the expectations of HAH 2.0. The only thing that is unrealistic about care plans for PHCs is that management thinks we can do that extra documentation process and still make 10 unique member contacts each day.

They’ve got PHC’s working way beyond scope of practice. PHC’s were originally instructed to identify needs and refer to proper area - care coordination - “pass the ball.”

It is unrealistic to assume PHC’s will be at a 10 a day metric with care plans properly completed and member needs sufficiently addressed. That’s nuts.

I'm a PHC and my team wasn't told anything about care plan training being changed. We are still being told that we begin the CBT training the week of 11/13 and so on according to the schedule that was given in the Live Healthy town hall meeting earlier this month.

In the last town hall it was announced roll out for care plans would be December 4th. Now being told early 2018.
