WHy did termination packet not include Cobra enrollment form?

I don’t get this. It was my understanding that Cobra enrollment form will be in the termination package I received on Tuesday. That was not the case and so I called 3rd party which told me that it will arrive in 7 to 10 business days. Now note that medical, dental, vision insurance expired on Niv 1and so temporarily I have no insurance starting on Wednesday. I am told that benefits will be retroactive to Nov 1 BUT when doctor tries to bill them we are out of coverage until proper paper work is done and benefits are reinstated.

There can be huge confusion if you need coverage in between the beuracacy.

Luckily there is way out of this unnecessary temporary lapse in coverage.

  1. Send email to benefits at Oracle Andy request for the enrollment form (there was some mention of this in the package).

  2. Benefits Dept sent me a personalized enrollment form to me in ~ 4 business hours.

  3. I checked off coverage, signed, and scanned form and email it to the 3rd party.

  4. An hour letter got email back stating that benefits are reinstated back dating November 1st.

If the personalized form were already included in the package then we could have had cobra coverage the next day on Nov 1. Obviously the benefits Dept knew how to generate it and so it escapes me why they did not include it in the package itself and make everyone’s life easy.

Yes, Oracle pays for one month but ONLY if you sign up for COBRA which you can not do without the required enrollment form. Until you check off boxes on the form, sign and return it to 3rd party you are out of coverage (I even got email from VSP with their list of plans I can sign up for). Once paper work is done it is back dated but if you need medical service in between then there will be confusion, they may ask you to pay full, rebill to get coverage etc.

In my particular case I am out of town starting tomorrow for two weeks and so if I wait for their form in mail then I won’t have coverage temporarily until end of the month. They did say I have up to 60 days to sign up for COBRA and it can still be back dated.

On the positive side, the 3rd party accepted scanned copy and reinstated my coverage in an hour.

I don’t think division matters but I was in old systems group 😊

You get termination packet ... then you get cobra 30 days later... oracle covers 1 month for you. When did you get packet and what division are you in?

Because oracle is f-- up and doesn’t care about when you get the forms, plus they use a very small company to handle this and doesn’t give this form enough notice to put the info together.
