I'm curious if anyone has any reliable information they have heard on this program actually closing in 2018. of course I know things do not look good for Humana at home based on the continued layoffs but wondered if any clinical care leaders or coaches knew actual information

IF FCMs will be doing skilled care (med rec, vitals) with LTIH then how do FCMs who are SW fit into the plan? Does anyone know if the plan is still to move to network and away from associates?
LTIH would be staying, it could be seen as the basis of the kindred transition...the staff are already in the home, doing the job..so instead of contracting out through senior bridge, bill Medicare under home health...LTIH may be secure

HCM RN here. I got several new members today that were moved out of LTIH. I read in one of the prior case notes that the member was notified that the program was being "restructured" What is going on? These members live in a large metropolitan area. Did I miss a memo? Is LTIH gone or just downsized? And, why am I still getting alerts on other members to move them to LTIH? Wouldn't it make sense to check it there is a LTIH HCM available prior to sending an alert?
I am a HCM RN not affected yet. I had a member that I referred to LTIH and got them back. Note saying a Care Manager was not available in the members area. I think that is very strange in a big city.....

CCL here. The closest thing I have to actual information is that DMs position is being back-filled. So there’s a plan to keep the program going at least for now.
