I am a former Sam’s Club associate and I want to apply for a job at a fulfillment center. Is the fact that I was a former associate a pro or a con for getting a job in the eyes of the managers? Any advice?

It is a definitie pro. Where I live the medical insurance is much better than at Sams, crazy I know being same company. PTO is different, holidays are paid and dont come out of your PTO balance. It looks like a Sams inside pretty much. Having worked for Sams is a huge plus. I almost transferred and it seems so laid back.
Why did you leave? Most people leave due to excessive absences. That is an issue.
I don't see it hurting your chances. I think they will just go with whoever is most qualified either way.
